NFHS Sports Live Stream

We are very excited to announce that the Bangor Public Schools Athletic Department has partnered with FinalForms, an online forms and data management service. FinalForms allows you to complete and sign athletic participation forms for your students. The most exciting news is that FinalForms saves data from season-to-season and year-to-year, meaning that you will never need to enter the same information twice! FinalForms also pre-populates information wherever possible, for each of your students, saving you time. In addition, you will be able to print the MHSAA  physical examination form directly from your FinalForms site and take that with you to the doctor for your child’s physical (no need to pick one up from school).

You may review your data at any time to verify it is current. You will be required to sign your forms once per year and after any update.

Click on the Parent Playbook to get started in FinalForms. If you require any support during the process, scroll to the page bottom and click “Use Support”.

We are asking that ALL parents of Bangor HS and Bangor MS athletes register their athletes on FinalForms.  Athletes MUST be registered and all required items completed on Final Forms to participate in athletics.  Fall practices begin August 9th.

Please register at and follow the prompts to create your account, create your students and sign your forms.

Thank you for your assistance in streamlining our paperwork processes at Bangor Public Schools.

Athletic Handbook

Physicals and MHSAA Sports Health Questionnaire

If you plan to participate in athletics this year and did not have a physical done last year, you will need to have one done prior to participating.  You can make an appointment with the health center located in the middle school by calling 427-6810.

If you had a physical last year, you will need to fill out the MHSAA Sports Health Questionnaire, or pick up one from the inside front door of the high school office.

Fall COVID-19 Athletic Policies for Bangor Public Schools Spectators (Home or Away Contests)

Per MHSAA guidelines, winter sports will only be allowed 2 tickets per athlete.  Bangor Public Schools will begin charging admission for all spectators:  Adults $5.00.  Students $2.00. Masks MUST be worn by every spectator and athlete.  Concession stands will remain closed at this time.
